While preserving natural teeth is always the priority, there are situations where extraction becomes necessary for the long-term health of the mouth. A surgical extraction is required when a tooth cannot be removed with simple forceps alone — this includes impacted wisdom teeth, teeth broken at the gumline, roots with complex anatomy or teeth associated with cysts or pathology. The procedure is performed under local anaesthesia (and sedation if requested) for complete comfort. The dentist or oral surgeon makes a small incision in the gum tissue to access the tooth, sections the tooth if necessary for easier removal and carefully extracts the roots with minimal trauma to the surrounding bone. It is indicated when a tooth is too damaged, decayed or weakened to be restored with a filling alone — such as after a root canal, a severely cracked tooth or a large, failing restoration. Crowns protect the tooth from further breakdown while restoring its natural appearance and full biting force. A dental bridge (fixed partial denture) replaces one or more missing teeth by anchoring to adjacent teeth (abutments) with crowns, suspending artificial teeth (pontics) in the gap. A removable partial denture (RPD) is a prosthetic device used to replace one or more missing teeth in patients who still retain some natural teeth. Unlike a complete denture, an RPD is designed to integrate with the remaining teeth, using them as anchors (abutments) for support and stability while replacing the absent ones. RPDs are constructed from a combination of a metal alloy framework (typically cobalt-chromium) or flexible acrylic, with acrylic teeth and gum-coloured bases. Metal framework RPDs are particularly valued for their strength, accuracy of fit and longevity. Flexible (valplast) RPDs offer a metal-free, aesthetic alternative with excellent comfort and gingival-coloured clasps that are nearly invisible. RPDs improve chewing efficiency, prevent the remaining teeth from drifting or tipping into gaps, maintain the vertical dimension of the bite and restore a natural-looking smile. They are a cost-effective and time-efficient solution, particularly when implants are not possible or desired and can be modified if future teeth are lost.
Certain oral habits, when persistent beyond a certain age, can significantly disrupt the normal development of teeth, jaws and facial structures. The most common of these include thumb/finger sucking, tongue thrusting, mouth breathing and nail biting. If not addressed, these habits can lead to open bites, protrusion of front teeth, crossbites and narrowing of the dental arches. Habit breaking appliances are custom-fabricated devices — either fixed or removable — designed to discourage the repetition of these habits in a gentle, non-punitive way. For thumb sucking, a palatal crib or rake is commonly used: a small, smooth appliance attached behind the front teeth that makes sucking unsatisfying without causing discomfort. For tongue thrusting, appliances that retrain the tongue's resting and swallowing position are employed. These appliances are typically used in children between the ages of 4 and 12, during the window when the habit is most likely to cause lasting dental changes. Early intervention is key — correcting the habit before significant skeletal changes occur avoids the need for more complex orthodontic treatment later.
